Journal The Alberta Children’s Hospital Family Journal
is a simple tool to help keep a record of your child’s
health journey. It’s a journal you can use to
write valuable information such as clinic visits, treatments,
medications, test results, resources, physicians and specialists.
The Family Journal can be downloaded and printed. Copies are also available through the Family & Community
Resource Centre and ambulatory clinic nurses.
